Documents to present:

     Identity card of the appearing party.
     Notebook for Food Products.
     Sworn Declaration of Medical Diet, signed and coined by the Public Health Assistance Center that issues it and they must state the pregnancy time that the pregnant woman is 26 weeks or more.
