To process a diet you must present:

     Notebook for Food Products.
     Model 12 Sworn Declaration of Medical Diet, duly signed and coined by the Public Health Assistance Center that issues it.
     The renewal of the medical diet may be processed by the consumer, any day of the month in which the current diet expires.
